Patient Forms and Procedures
We recognize that each patient is an individual and has unique health care needs. We value every patient and are guided by our commitment to provide effective treatment and compassionate care to our patients. Bay Area Hyperbarics is a place where the purpose is not only to heal your body but also to support you and your loved ones during the process.
In order to begin Hyperbaric Oxygen Therapy (HBOT) at Bay Area Hyperbarics and to ensure your experience is a smooth one, please complete the following steps prior to the start of your treatment:
Step 1: Obtain a prescription from your physician or our Medical Director for Hyperbaric Oxygen Therapy. Our Medical Director is available for consultations and medical evaluations, including writing a prescription for HBOT.
Step 2: Download, print, fill out, and sign all of the required Patient Intake Forms, then bring them with you when you come for your appointment or fax them to us ahead of time.

If you do not have Adobe Reader, you can obtain it free of charge at http://www.adobe.com (once on the Adobe site, click on the "Get Adobe Reader" button).Step 3: Call to speak with our Billing Specialist to determine the cost of your treatment and to discuss any insurance related items.
Step 4: Call to speak with one of our Hyperbaric Technicians to schedule your orientation and first treatment appointment.

One of our objectives is to inform you of what to expect during your hyperbaric treatments with us. Prior to your first treatment you will be asked to come in early to go through an orientation. The purpose of this orientation is to answer any questions you may have, to review your completed Patient Intake Forms, and to discuss how your daily routine will go. Please review, fill out, and sign all our medical Patient Intake Forms ahead of time and bring them with you. Feel free to ask any questions; we are here to help you.
A Few Do's and Do Not's
- For your safety and the safety of those around the chambers, nothing can be taken into the chamber with you, unless approved by our Certified Hyperbaric Technologists.
- The clothing worn in the chambers must be made out of 100% cotton. No metal in the form zippers, snaps, etc. can be on the clothing. We provide this clothing for you.
- No make-up, hairspray, gels, lotions, colognes, perfumes, wigs or hairpieces should be worn during your hyperbaric treatments.
- It is recommended that you eat one hour prior to your treatment.
- Avoid caffeine, alcohol, and nicotine before and after your treatment.
- For the privacy and confidentiality of all our patients, no family member or friends are allowed in the treatment area, unless pre-approved by one of our Certified Hyperbaric Technologists.

Scheduling
Every attempt will be made to plan your treatments so that they fit your schedule. Once your time slot is established, we try it keep it reserved for you for the duration of your treatments. In the event you begin to miss appointments, you are unable to come in, or we have an emergency patient, we may need to assign your time slot to another patient. Keep in mind that generally, time slots are on a first come first serve basis. As patients complete their treatment plan a more desirable time slot may open up, in which case we can offer it to you.
Please arrive 15 minutes prior to your scheduled treatment time in order to change your clothing and prepare for your treatment. Our daily treatment schedule runs smoothly if patients arrive on time. If you are late, your treatment time may have to be shortened. A $35 fee may apply if you cancel your appointment less than 24 hours in advance or if you are a "no show" for your appointment.
