We recognize that each patient is an individual and has unique health care needs. We value every patient and are guided by our commitment to provide effective treatment and compassionate care to our patients. BayArea Hyperbarics is a place where the purpose is not only to heal the body but also to support the patient and collaborate with his/her medical provider during the process.
For your patient to begin hyperbaric oxygen therapy (HBOT) at BayArea Hyperbarics and to ensure your patient’s experience is a smooth one, please complete the following steps:
Physician Referral Procedure
- Write a prescription for your patient for Hyperbaric Oxygen Therapy (HBOT). This RX should include the patient’s name and diagnosis, the physician’s signature, license number, date, physician’s name, office address, phone, and fax. This prescription can be faxed to us at (650) 567-9186 or sent with the patient.
- Fax or send us the patient’s medical chart notes at the same time as the prescription. The medial chart notes are critical for us to have in order to facilitate the insurance claim processing for the patient. Fax: (650) 567-9186
- Call us to schedule a first appointment for your patient or provide us with the patient’s name and phone number; we are happy to work with the patient directly to schedule his/her treatment. You may also have the patient call us directly at (650) 567-9110.
It is our standard procedure to send or fax the referring physician a memo summarizing the patient’s hyperbaric oxygen therapy at the conclusion of the patient’s treatment plan.
