Every surgery creates a wound, and every wound needs oxygen to heal. When surgical sites don't get enough of it — due to swelling, reduced blood flow, or tissue damage — the result can be infection, delayed healing, or wound breakdown that requires additional treatment. Hyperbaric oxygen therapy (HBOT) is increasingly used alongside standard surgical care to help reduce exactly these complications.
Why Oxygen Matters After Surgery
Surgical wounds are especially vulnerable in the days and weeks after a procedure. According to a structured review published through the National Center for Biotechnology Information (NCBI), tissue hypoxia — low oxygen at the wound site — is a central driver of surgical complications, including infection, wound separation, and flap or graft failure. HBOT works by dramatically increasing the amount of oxygen dissolved in blood plasma, allowing it to reach compromised tissue that would otherwise struggle to heal.
What the Research Shows
The evidence for HBOT's role in surgical recovery spans multiple specialties:
Surgical site infections: A narrative review published in the journal Medicina found that HBOT can support rapid healing in surgical site infections following cardiac, spinal, and urogenital procedures, working through direct antibacterial effects and by strengthening the immune system's ability to clear infection.
Reconstructive and flap surgery: Research reviewed by the NCBI found that HBOT improves flap and graft survival, including in irradiated or poorly perfused breast reconstruction tissue, reducing wound breakdown and the need for revision surgery.
Head and neck surgery: A study of patients with non-healing wounds after surgery in previously irradiated tissue found that 12 of 15 patients healed completely with HBOT, compared to just 7 of 15 in a comparable group that didn't receive it.
Spinal surgery: A retrospective study of high-risk neuromuscular scoliosis patients found that adding HBOT to standard antibiotic prophylaxis was associated with fewer deep postoperative infections.
How It Works
HBOT is thought to support surgical recovery through several connected mechanisms: increasing oxygen delivery to healing tissue, encouraging new blood vessel growth (angiogenesis), supporting collagen production for wound strength, reducing swelling, and directly inhibiting the anaerobic bacteria that often cause surgical site infections.
Is It Right for Every Surgery?
HBOT isn't necessary for most routine, low-risk procedures that heal normally. It's most often considered for patients at higher risk of complications — including those with diabetes, prior radiation exposure, smoking history, or surgeries involving compromised blood flow, flaps, or grafts. Researchers are still working to standardize exactly which patients benefit most, and results should be discussed with your surgical team.
